Welcome to Tier 1 Chiropractic Concierge.


These Terms & Conditions establish the expectations, responsibilities, and agreements between you (“the client”) and Tier 1 Chiropractic Concierge (“the provider,” “we,” or “us”).

By booking any appointment — including complimentary consultations, single visits, or membership-based visits — you acknowledge that you have read, understood, and agree to the following terms.

1. Scheduling & Appointments

  • All appointments must be scheduled through our online booking system.

  • Treatment is provided exclusively at the location you designate (home, office, or private gym).

  • You must ensure a safe, private environment suitable for care.

  • A minimum clear space of approximately 6 ft x 3 ft is required for the chiropractic table.

  • Entry instructions (gates, parking, building access) must be provided before the appointment time.

2. Arrival Window for Mobile Care

We operate with a 10–15 minute arrival window due to:

  • traffic

  • parking

  • community gate access

  • previous appointments running slightly over

  • travel efficiency

This is standard for mobile healthcare providers. You will be notified if arrival is expected outside this window.

3. Cancellation & Rescheduling Policy

To respect your provider’s time and maintain schedule availability:

  • Appointments must be canceled or rescheduled with at least 6 hours’ notice.

  • Late cancellations may result in a forfeited session.

  • Membership sessions missed due to late cancellation do not roll over to the next billing cycle.

4. Membership Terms (Tier 1 & Tier 2)

By subscribing to a membership:

  • Memberships bill automatically every 30 days.

  • Included sessions must be used within the billing cycle and do not roll over.

  • You may cancel anytime; cancellation becomes effective at the end of the current cycle.

  • Additional sessions beyond your membership allotment may be offered at an adjustment monthly fee.

  • Membership benefits apply only to the individual subscriber unless otherwise stated.

  • Missed or unused visits cannot be refunded or credited.

5. Scope of Services Provided

Tier 1 Chiropractic Concierge provides chiropractic and related therapeutic services, which may include:

  • Spinal adjustments

  • Joint mobilization

  • Soft-tissue therapy

  • Active Release Technique (ART)–style manual therapy

  • Cupping

  • Instrument-Assisted Soft Tissue Mobilization (IASTM/scraping)

  • Percussive therapy (massage gun)

  • E-stim or muscle stimulation modalities

  • Shockwave therapy (portable unit)

  • Stretching & mobility work

  • Corrective exercise

  • Recovery-based modalities

All treatments are customized based on clinical judgment and your goals.

6. Clinical Consent & Informed Consent for Manipulation

By booking, you consent to chiropractic evaluation and treatment, which may include:

  • manual joint adjustments

  • joint mobilization

  • soft-tissue techniques

  • stretching

  • pressure-based techniques

  • therapeutic exercise

  • the use of modalities listed above

You understand that:

  • Chiropractic care is generally safe, but risks may include soreness, stiffness, or temporary discomfort.

  • Severe adverse reactions are rare but may include fractures, ligament injury, or neurological changes.

  • You may refuse or discontinue any part of treatment at any time.

  • Results are not guaranteed and vary from person to person.

  • You must inform your provider of any new symptoms before treatment.

7. Contraindications to Treatment

You agree to notify the provider of any of the following, which may require modification or referral:

  • acute injury

  • fracture

  • recent surgery

  • severe osteoporosis

  • active infection

  • uncontrolled hypertension

  • cancer history with active symptoms

  • bleeding disorders

  • anticoagulant use

  • pregnancy (for certain techniques)

Your provider will adjust or defer treatment based on clinical safety.

8. Red Flag Symptoms

The provider may postpone care and recommend referral if you report:

  • unexplained weight loss

  • night sweats or fever

  • loss of bowel or bladder control

  • saddle numbness

  • progressive neurological weakness

  • severe night pain

  • recent major trauma

These signs may require urgent medical evaluation.

9. Liability Waiver

By booking care with Tier 1 Chiropractic Concierge, you acknowledge and agree that:

  • You assume all risks associated with chiropractic treatment.

  • You understand the nature and purpose of the procedures.

  • You release Tier 1 Chiropractic Concierge and its provider(s) from liability for injuries except those caused by gross negligence or misconduct.

  • You agree that treatment is voluntary and can be declined at any time.

  • You are responsible for providing a safe environment for in-home care.

10. Consent to Mobile Treatment

By booking, you confirm:

  • You understand treatment occurs outside a traditional clinic setting.

  • You accept responsibility for ensuring the space is safe and appropriate.

  • You consent to receiving chiropractic care in your chosen environment.

  • The provider may refuse care if the space is unsafe or unsuitable.

11. Photography & Video Consent (Optional)

If you choose to participate in photographs or recorded content:

  • You will be asked directly for permission.

  • You may decline at any time.

  • Content will never be recorded or used without your explicit consent.

  • You may revoke consent for future content at any time.

(If you do not explicitly consent, no media will be captured.)

12. Family or Household Care

  • Family members living in the same household may schedule back-to-back sessions.

  • Each individual requires their own intake form and consent.

  • Memberships apply to one individual per subscription unless a designated family membership is offered.

13. Communications Policy

Members may contact the provider via text or phone for:

  • appointment questions

  • updates on symptoms

  • clarification regarding home care

  • general guidance

You agree not to use communication for:

  • emergencies

  • urgent medical concerns

  • ongoing clinical assessment that requires in-person evaluation

Emergency concerns should always be handled by calling 911 or seeking immediate medical care.

Response times may vary but are typically within business hours.

14. Payment, Fees & Refund Policy

  • Payment is collected at booking unless covered by membership credit.

  • All fees are final and non-refundable.

  • No refunds are issued for used, unused, or forfeited membership sessions.

  • Late cancellation or no-show fees, if applied, are non-refundable.

  • Tier 1 Chiropractic Concierge does not submit insurance claims.

  • Superbills may be provided upon request, but reimbursement is not guaranteed.

15. Privacy & Confidentiality

All personal health information is kept confidential according to professional standards and used solely for:

  • clinical care

  • scheduling

  • communication

  • administrative purposes

Your information will never be shared without consent unless required by law.

16. Agreement & Acknowledgment

By booking any appointment, you confirm that:

  • You have read and understand these Terms & Conditions.

  • You agree to all policies outlined above.

  • You voluntarily consent to chiropractic evaluation and treatment.

  • You understand the scope, risks, and limitations of mobile chiropractic care.

  • You understand the membership structure and cancellation policy.

  • You accept full responsibility for providing a safe environment for care.
